Fatal Facts is a publication that is primary produced for coroners although it is now also available to the general public.

Fatal Facts identifies and summarises all coronial recommendations that have been upload to the NCIS over the relevant period. It also provides in depth case studies on three to four cases where recommendations were made. Finally the publication includes a list of recent information requests that have been performed by the NCIS, and some tips for using the NCIS.

A general edition of Fatal Facts is published quarterly, and special editions focusing on particular subject area are produced as required.

Edition 14 - September 2007

The common topic area featured in Edition 14 concerns product related deaths.

Edition 13 - June 2007

Common topic areas featured in Edition 13 include level crossing fatalities, deaths related to the prescription of medication and deaths of children/unauthorised personnel on work sites.

Edition 12 - April 2007

The common topic area featured in Edition 12 relates to aircraft crashes.

Edition 11 - November 2006

Common topic areas featured in Edition 11 include falls from balconies, rock-fishing related deaths and overlaying/co-sleeping deaths of children.

Edition 10 - July 2006

Common topic areas featured in Edition 10 include work-related deaths and bicycle-related deaths.

Edition 9 - May 2006

Common topic areas featured in Edition 9 include deaths during underwater recreational diving, fatalities due to being struck by trains or trams at crossings, and intentional self-harm deaths of persons with mental health problems.

Edition 8 - March 2006

Common topic areas featured in Edition 8 include police pursuits, deaths in custody, child run overs and interpersonal (domestic) violence.

Edition 7 – July 2005

Common topic areas featured in Edition 7 include motor vehicle accidents, agricultural/industrial equipment, drowning, and the treatment of persons in care whilst inebriated or drug affected.

Edition 6 – January 2005

Common topic areas featured in Edition 6 include transport related deaths, recreational boating, and adverse event relate deaths in hospitals.

Editions 1-5 of Fatal Facts are only available in hard copy. Please contact NCIS should you wish to obtain copies.

House Fires Bulletin

This publication (produced by the NCIS team) outlines some trends and patterns extracted from the NCIS database where an individual has died as a result of a residential fire.

Product Related Bulletin

This publication (produced by the NCIS team) provides an overview of deaths contributed to by a faulty, misused or poorly maintained consumer product (as identified on the NCIS database).

A regular newsletter that provides information about the operations of the NCIS, any interface changes, progress towards data completeness and timeliness and any other issues of general interest for users (previously known as MUNCCI Talk).

The Coding Manual provides a step-by-step explanation for the coding and completion of a selection of data fields. The manual provides practical instructions, examples of specific scenarios and guidelines for the entry of more complex and difficult core data items. For more information refer to Data Quality.

• Practical tips for using the NCIS – an abbreviated version of the full search manual which highlights the basic search mechanisms of the NCIS, provides sample searches, and tips to assist users
• Limitations of the NCIS - documents things that users of the NCIS should take into account when conducting searches

An electronic copy of the NCIS brochure which provides an overview of the system, the data elements available and contact details.

An initial document which provides information about some of the benefits of the NCIS for coroners and practical uses. Regular updates will be produced to highlight some other practical uses of the system as they occur.
