Fatal Facts is a publication that is primary produced for coroners although it is now also available to the general public.
Fatal Facts identifies and summarises all coronial recommendations that have been upload to the NCIS over the relevant period.
It also provides in depth case studies on three to four cases where recommendations were made. Finally the publication includes
a list of recent information requests that have been performed by the NCIS, and some tips for using the NCIS.
A general edition of Fatal Facts is published quarterly, and special editions focusing on particular subject area are produced
as required.

The common topic area featured in Edition 14 concerns product
related deaths.

Common topic areas featured in Edition 13 include level crossing
fatalities, deaths related to the prescription of medication and
deaths of children/unauthorised personnel on work sites.

The common topic area featured in Edition 12 relates to aircraft
crashes.

Common topic areas featured in Edition 11 include falls from
balconies, rock-fishing related deaths and overlaying/co-sleeping
deaths of children.

include deaths during underwater recreational diving, fatalities due
to being struck by trains or trams at crossings, and intentional
self-harm deaths of persons with mental health problems.

include police pursuits, deaths in custody, child run overs and
interpersonal (domestic) violence.
Edition 7 – July 2005
Common topic
areas featured in Edition 7 include motor vehicle accidents,
agricultural/industrial equipment, drowning, and the treatment of
persons in care whilst inebriated or drug affected.

Common topic
areas featured in Edition 6 include transport related deaths,
recreational boating, and adverse event relate deaths in hospitals.

Fires Bulletin
This publication (produced by the NCIS team) outlines some trends
and patterns extracted from the NCIS database where an individual has
died as a result of a residential fire.
Product Related Bulletin
This publication (produced by the NCIS team) provides an overview
of deaths contributed to by a faulty, misused or poorly maintained
consumer product (as identified on the NCIS database).
A regular newsletter that provides information about the operations of the NCIS, any
interface changes, progress towards data completeness and timeliness and any other
issues of general interest for users (previously known as MUNCCI Talk).
